The SAFE Passage
Transitional Living Program is a community-based program for homeless youth,
ages 18 to 21, who are in need of housing, and educational, emotional, and
fiscal support. SAFE Passage provides safe housing, food and clothing,
confidential case management and aftercare services, as well as instruction in
independent living skills, all at no cost to homeless youth. |

The
Program's educational component is designed to aid youth in making a smooth
transition into a stable independent living situation. Education is provided in,
but not limited to, the following areas: interpersonal skills; personal
appearance and awareness; educational planning; job seeking and maintenance
skills, including resume and cover letter writing; money management, budgeting
and consumer awareness; housing; housekeeping, including household and time
management; healthy food preparation and storage; health, including STD/HIV
prevention; positive parenting and pregnancy prevention (as appropriate);
domestic violence; emergency and safety skills; community resources; legal
skills; and transportation. |

The
anticipated benefits of the program are far-reaching. Participants will begin to
change some negative attitudes and behaviors, to place a higher value on the
family, and accordingly, to promote responsible independent living, which
includes positive parenting and family-oriented behaviors. Specifically, it is
anticipated that implementation of services shall result in clients placing a
higher value on stable and nurturing home environments, healthy child
development, and prompt and appropriate resolution of crises. |

The Application Process is as follows:
All referrals shall require an in-depth, face-to-face assessment before a
decision can be made regarding the appropriateness of a person for receipt of
services. At this time the client will be provided with an Initial Application
form and a Criminal Background Check (CBC) Information Disclosure form. After
the CBC has been run and the application processed, the staff will determine
whether or not the youth is appropriate for admission. If it is decided that the
applicant is appropriate, the youth will be contacted and a formal intake will
be scheduled. |

In
general, the Transitional Living Program shall not accept for admission those
persons who have a current presentation of, or within the past 12 months have a
history of: |
 |
aggressiveness or violence (e.g., assault or battery); |
 |
severe or chronic mental illness; |
 |
suicidal intentions, gestures or acts; |
 |
noncompliance; |
 |
serious criminal charges not resolved by court action; |
 |
placement pending in another agency or home; |
 |
severe or chronic alcohol or other drug addiction, and
|
 |
being on probation or parole |
|
Exceptions to these guidelines may be
possible, given individual circumstances. Exceptions may be approved only after
consultation with the Program Coordinator. For siblings and friends in crisis,
housing can be offered provided all other admission criteria have been satisfied
and with prior approval of the Program Coordinator.

WISCONSIN Homeless Management Information SySTEM
This program receives funding from U.S. Department of Housing and
Urban Development and/or the State of Wisconsin Department of
Commerce to provide services for homeless and near homeless
individuals and their families. A requirement of this funding is that
this agency participates in the Wisconsin Homeless Management
Information System. This system collects basic information about
clients receiving services from this agency. This requirement was
enacted in order to get a more accurate count of individuals and
families who are homeless and to identify the need for different
services.
We only collect and share only
the information that we consider to be appropriate. The collection and
use of all personal information is guided by strict standards of
confidentiality. A copy of our Privacy Notice describing our privacy
practice is available to all consumers upon request.
Your information will be shared
with other area agencies that participate in the network unless you
request that your information remain confidential and be only
available to case workers and intake workers within this
agency. Please talk to a case manager or intake worker about keeping
your records closed if you are interested.
